Statins, including popular medications like Lipitor (atorvastatin) and Crestor (rosuvastatin), are among the most commonly prescribed drugs in the United States. Their ability to lower LDL cholesterol (the “bad” type) has undoubtedly saved countless lives by reducing the risk of heart attacks and strokes.
However, as a pharmacist with more than 20 years of experience, I’ve seen firsthand that every powerful medication comes with trade-offs. One of the most overlooked consequences of long-term statin use is the depletion of a crucial nutrient in the body: Coenzyme Q10 (CoQ10).
What is CoQ10?
CoQ10 is an antioxidant and an essential component of the energy production process in your cells. It is particularly abundant in organs that require a lot of energy, such as the heart and muscles, and it has benefits for everything from heart health to fertility to ridding the body of harmful substances like free radicals.
How Statins Deplete CoQ10
Statins work by inhibiting an enzyme in the liver called HMG-CoA reductase, which plays a critical role in the production of cholesterol. While this is beneficial for lowering LDL cholesterol, the same enzyme is also vital for producing CoQ10. This unintended side effect leads to a significant reduction in CoQ10 levels throughout the body. And when statins reduce CoQ10 levels, it can lead to a cascade of problems, especially for people already at risk of deficiency due to aging, chronic illnesses or other medications.
Symptoms of CoQ10 Deficiency
The symptoms of CoQ10 deficiency can range from mild fatigue to debilitating muscle and heart problems. In extreme cases, I’ve seen patients experience conditions that dramatically impact their quality of life. Here are some of the more severe manifestations of CoQ10 depletion.
Debilitating Muscle Pain and Weakness (Myopathy)
Patients often describe severe muscle cramps, weakness, or even an inability to climb stairs or perform everyday activities. For some, this pain can mimic that of advanced arthritis or fibromyalgia, causing significant distress.
Heart Dysfunction
Since CoQ10 is critical for heart muscle energy production, its depletion can exacerbate conditions like heart failure. I’ve encountered patients whose fatigue and breathlessness were so profound that they struggled to walk across a room.
Neurological Issues
CoQ10 also plays a role in protecting brain cells from oxidative stress. Some individuals report memory issues, confusion, or difficulty concentrating, which can resemble early signs of dementia.
Chronic Fatigue
The lack of cellular energy leads to exhaustion that no amount of sleep seems to alleviate. This symptom alone can profoundly affect a person’s ability to work, care for family, or enjoy life.
The Best Way to Supplement and Prevent Deficiency
To mitigate the effects of CoQ10 depletion, I recommend supplementation, particularly for patients on long-term statin therapy. Here’s what you need to know about CoQ10 supplementation.
Form Matters
Look for supplements containing ubiquinol, the active form of CoQ10. While ubiquinone (the oxidized form) is more common, your body must convert it to ubiquinol to use it effectively. Ubiquinol is more bioavailable, especially for older adults.
Dosage
Typical doses range from 100 to 300 mg daily, depending on the severity of symptoms and individual needs. For patients experiencing significant muscle pain or fatigue, starting at a higher dose and adjusting based on response may be necessary. Work with your doctor and/or pharmacist to find the one that works for you.
Timing
Taking CoQ10 with a meal containing fat can enhance absorption. Consider splitting the dose to twice daily for sustained benefits.
Consistency Is Key
CoQ10 isn’t an overnight fix. It may take several weeks to months of regular supplementation to notice improvements. Encourage patience and adherence.
A Proactive Approach to Statin Use
As pharmacists, we have a responsibility to look beyond the medication bottle and consider the whole patient. If you’re taking a statin, talk to your health care provider about CoQ10 supplementation. Not only can it help prevent or alleviate the symptoms of deficiency, it can also enhance your overall quality of life.

Statins are powerful tools in the fight against cardiovascular disease, but they work best when paired with strategies to protect the body’s natural balance. Don’t overlook the small changes – like adding a CoQ10 supplement – that can make a big difference in your health journey.
